• a copy of the letter confirming ownership, signed by a band council representative

Proof of purchase

An electronic copy of the purchase invoice for the efficient pump.

The invoice must include:

• the invoice number
• the invoice date
• the customer name
• the pump installation address
• the retailer's name
• the retailer's GST/QST numbers
• the product description, indicating whether it is a two-speed or variable-speed model
• the purchase cost
